Supported Living Services Facilitator II
DEFINITION:  Through communication, planning, training, and implementation, oversees

the consumer’s IPP objectives with respect to Supported Living Services with an emphasis on

consumer health, safety, and quality of life.
LOCATION: Greater Sacramento Area                  
STATUS: Exempt                                                              REPORTS TO:  Program Manager
SPECIFIC DUTIES AND RESPONSIBILITIES: 

  • Experience with Training, scheduling, and supervising direct service staff (more than 20 employees).
  • Excellent verbal and written communication skills
  • Excellent organizational skills
  • Act as a liaison with outside agencies
  • Ability to coordinate the implementation of client services and goals.
  • Allocate and manage IHSS hours and ensure proper usage, including direct service and billing.
  • Submit all paperwork to office and/or external agencies as deadlines require.
  • Work toward the resolution of these opportunities and problems.
  • Other duties as assigned

QUALIFICATIONS: 

  • Bachelor’s Degree preferred
  • Valid California Driver’s License.
  • Willingness to work a flexible schedule.
  • Reliable transportation with proof of insurance


SLS Direct Service Trainer - Caregiver

Position Description: Provide direct service to clients. Assist in goals related to Supported Living.
Openings available for a variety of shifts including:
day, swing, evenings and overnights 
(PT/FT and weekends) 

Job Duties:

  • Assist in determining clients goals with regard to Title 17 “Supported Living Services”
  • Assess knowledge, skills, and abilities of client(s) in functional areas: household maintenance, meal planning and preparation, shopping, money management, relocation, socialization and recreation, health & safety, communication & problem solving, accessing medical & other community resources.
  • Provide training and assistance, including personal care as required, in areas of deficiency to meet the clients’ goals.
  • Establish training or maintenance schedules with clients.
  • Participate in all meetings as needed.
  • Maintain all necessary records for services rendered.
  • Submit all paperwork to office as required.
  • Immediately bring to the attention of the team and/or supervisor any opportunity for improvement in agency processes, and/or problem affecting the agency or its services to clients.
  • Work toward the resolution of these opportunities and problems.
  • Other duties as assigned.

Job Requirements:

  • Ability to learn and retain new skills as required in a timely manner.
  • Knowledge of necessary skills required for an adult to live in a supported living environment.
  • Ability to respect the rights and dignity of all clients.
  • Valid California Drivers License.
  • Willingness to work a flexible schedule.
  • Reliable transportation with proof of insurance.
  • All DSPs must work at least one weekend shift.


Independent Living Skills Facilitator
Position Description: Through assessment, training, and support, facilitate self-directed outcomes for adults with developmental disabilities. Identify and strengthen the alliances, commitments, and strategies that result in unique outcomes in the lives of the people served.
Job Duties:

  • Assist in determining clients’ goals with regard to Title 17
  • Assess knowledge, skills, and abilities of clients in functional areas: household maintenance, meals planning and preparation, shopping, money management, relocation, socialization and recreation, health & safety, communication & problem solving, accessing medical & other community resources.
  • Provide training and assistance in areas of deficiency to meet the clients’ goals.
  • Establish training or maintenance schedules with clients.
  • Maintain all necessary records for services rendered.
  • Participate in all meetings as needed.
  • Work in collaboration with Alta Regional Center and clients.

Job Requirements:

  • Knowledge of necessary skills required for an adult to live independently
  • Willingness to work a flexible schedule; may include nights and weekends.
  • Ability to respect the rights and dignity of all clients
  • Ability to learn and retain new skills as required in a timely manner.
  • Professionalism and problem-solving skills a must.
  • Valid California Driver License

Empowered Parenting Facilitator
Position Description: Through assessment, training, and support, facilitate self-directed outcomes for adults with developmental disabilities. Identify and strengthen the alliances, commitments, and strategies that result in unique outcomes in the lives of the people served.
Job Duties:

  • Assist in determining clients’ goals with regard to Title 17 or other state mandates.
  • Assess knowledge, skills, and abilities of clients in functional areas: early childhood development, understanding childhood handling anger, problem solving, setting boundaries, transitions, and safety.
  • Provide training and assistance in areas of deficiency to meet the clients’ parenting goals.
  • Establish training or maintenance schedules with clients
  • Participate in a meetings as needed
  • Maintain all necessary records for services rendered
  • Submit all paperwork to office as required

Job Requirements:

  • Ability to learn and retain new skills as required in a timely manner.
  • Knowledge of necessary skills required for an adult to live in an independent environment.
  • Ability to respect the rights and dignity of all clients.
  • Willingness to work a flexible schedule.
  • Your own reliable transportation with proof of insurance in your name.


Elder Care Services Provider
DEFINITION:  Provide direct service to assure the needs of the client are met.
STATUS:  Non-Exempt                              REPORTS TO:  Case Manager
SPECIFIC DUTIES AND RESPONSIBILITIES: 

  •  Assist in determining client goals and needs.  
  • Assess the knowledge, skills, and abilities of clients in functional areas:  household maintenance, meal planning and preparation, shopping, money management, relocation, socialization and recreation, health and safety, communication and problem solving, accessing medical & other community resources.
  • Develop and maintain relationship with family and/or support system of person served.
  • Assist clients in various activities of daily living (ADL) such as bathing, toileting, personal hygiene, etc
  • Prepare meals and snacks as requested.
  • Perform light housekeeping such as vacuuming, sweeping, mopping, laundry, and cleaning surfaces, etc.
  • Participate in all meetings as needed.
  • Maintain all necessary records for services rendered.
  • Submit all paperwork to office as required by supervisor and members of the management team.
  • Maintain a professional relationship with external agencies and support systems.
  • Immediately bring to the attention of the team and/or supervisor any opportunity for improvement in agency processes, and/or problem affecting the agency or its services to clients and work toward the resolution of these opportunities and problems.
  • Other duties as assigned.
